These recommended interventions aim to provide the best mix of preventive and wellness care programs, outpatient vs. in-patient, pre and post-discharge steps that are personalized for a person’s health status, its historical evolution over time, and its most likely course of evolution. By accessing our proprietary set of care management program recommendations and delivery capabilities, we help patients access constant support and stay healthy, while lowering the total cost of care to a health plan.
